Proper credentialing is key to maintaining our strong ethical and professional standards. We have a full-time staff dedicated to building our network and ensuring that all of our providers are properly certified to conduct functional capacity evaluations, post offer agility testing, and other mobile and physical assessments.

We ask that each facility have their therapists complete our credentialing form and submit it to us along with a copy of their CV or resume, a copy of their PT or OT license, and a sample FCE report. We also require a copy of the facility’s liability insurance declaration page (showing the liability aggregates) a W9 form, and our completed Provider Questionnaire. The credentialing paperwork should be returned either by email, to credentials@connectfce.com, or by fax to 215-860-8650. Once received, we conduct a background check, and if approved, a contract will be forwarded to the facility for review and signature.
